In 2005, an estimated 22.2 million persons aged 12 or older were classified with substance dependence or abuse in the past year (9.1 percent of the population aged 12 or older). Of these, 3.3 million were classified with dependence on or abuse of both alcohol and illegal drugs, 3.6 million were dependent on or abused illegal drugs but not alcohol, and 15.4 million were dependent on or abused alcohol but not illegal drugs.

As observed in prior years, the level of alcohol use was strongly associated with illegal drug use in 2001. Among the 12.9 million heavy drinkers aged 12 or older, 30.7 percent were current illegal drug users. For binge drinkers who were not heavy drinkers, 15.9 percent reported past month illegal drug use. Other drinkers (i.e., past month alcohol use but not binge drinking) had a rate of 5.4 percent for current illegal drug use, and persons who did not use alcohol in the past month were least likely to use illegal drugs (2.8 percent).

In 2008, 21.4 percent of teens aged 12 to 17 reported that, in the past year, they had gotten into a serious fight at school or at work; this was similar to the rates in 2007 (22.3 percent) and 2002 (20.6 percent). Almost one in six (14.5 percent) in 2008 had taken part in a group against group fight, which was lower than the rates in 2007 (15.4 percent) and 2002 (15.9 percent). About 1 in 30 (3.2 percent) had carried a handgun at least once, which was similar to the rates in 2007 and 2002 (both at 3.3 percent). An estimated 3.0 percent had sold illegal drugs, which was similar to the rate of 2.9 percent in 2007, but was lower than the 4.4 percent rate in 2002. In 2008, 4.6 percent had, at least once, stolen or tried to steal something worth more than $50; this was similar to the rates of 4.3 percent in 2007 and 4.9 percent in 2002. An estimated 7.3 percent had, in at least one instance, attacked others with the intent to harm or seriously hurt them in 2008, which was the same as the rate in 2007 and was similar to the 7.8 percent reported in 2002.

In 2006, adults aged 18 or older with MDE in the past year were more likely than those without MDE to have used an illegal drug in the past year (27.7 vs. 12.9 percent). A similar pattern was observed for specific types of past year illegal drug use, such as marijuana, cocaine, hallucinogens, inhalants, and the nonmedical use of prescription type psychotherapeutics.
