Only heroin, which is almost identical in its chemical structure to morphine, can be closely compared in dependence liability.

Among people who needed but did not receive illegal drug use treatment and felt they needed treatment (based on 2004-2005 combined data), the six most often reported reasons for not receiving treatment were (a) cost or insurance barriers (36.7 percent), (b) not ready to stop using (34.7 percent), (c) stigma (24.4 percent), (d) did not feel the need for treatment at the time or could handle the problem without treatment (16.3 percent), (e) did not know where to go for treatment (16.3 percent), and (f) other access barriers (16.1 percent).

The use of alcohol and tobacco, both legal substances, far outpaces the use of marijuana. This suggests that keeping marijuana illegal would reduce the availability and acceptability of the drug.

A recent survey by the National Institute on Drug Abuse at Columbia University indicated t Pharmaceuticals taken without a prescription or a doctor's supervision can be just as dangerous as taking illegal drugs or alcohol. Over half of primary care physicians reported that they would find it difficult speaking with their patients about substance abuse and addiction.

The increase in cigarette initiation was due primarily to an increase among persons initiating at age 18 or older. Between 2002 and 2008, the number of initiates under age 18 remained stable (1.3 million in 2002 and 1.4 million in 2008), but the number of initiates aged 18 or older increased from about 600,000 to 1 million.

Other extremely potent narcotics, such as hydromorphone and fentanyl, have a high abuse potential, but it is still considered to be less than that of morphine.
