Heroin users who inject the drug expose themselves to additional risks, including contracting human immunodeficiency virus (HIV), hepatitis B and C, and other blood-borne viruses.

The average age at first use among recent initiates aged 12 to 49 was 23.4 years in 2008. There were no significant changes in the number of initiates or in the average age at first use from 2007 to 2008. The number of heroin initiates was not significantly different from the number in 2002 (117,000).
In 2008, an estimated 20.1 million Americans aged 12 or older were current (past month) illegal drug users, meaning they had used an illegal drug during the month prior to the survey interview.
