In 2008, an estimated 2.9 million persons aged 12 or older used an illegal drug for the first time within the past 12 months. This averages to almost 8,000 initiates per day and is similar to the estimate for 2007. A majority of these past year illegal drug initiates reported that their first drug was marijuana (56.6 percent). Nearly one third initiated with psychotherapeutics (29.6 percent, including 22.5 percent with pain relievers, 3.2 percent with tranquilizers, 3.0 percent with stimulants, and 0.8 percent with sedatives). A sizable proportion reported inhalants (9.7 percent) as their first illegal drug, and a small proportion used hallucinogens as their first drug (3.2 percent).

Nationally in 2002, about 3.0 percent of persons aged 12 or older were dependent on or had abused illegal drugs in the past year. The District of Columbia had the highest rate of illegal drug abuse or dependence for persons aged 12 or older (4.3 percent), and Kansas had the lowest rate (2.5 percent). Most of the States in the highest fifth were in the West (six States). Only four of the States in the top fifth for illegal drug dependence or abuse also were in the top fifth for alcohol dependence or abuse among persons aged 12 or older: District of Columbia, Montana, New Mexico, and Rhode Island. Rhode Island had the highest rate for illegal drug dependence or abuse among persons aged 18 to 25 (13.9 percent); Vermont had the highest rate among teens (8.8 percent). There were no States common to the highest fifth for all three age groups (12 to 17, 18 to 25, and 26 or older).

There were 6.2 million (2.5 percent) persons aged 12 or older who used prescription type psychotherapeutic drugs nonmedically in the past month. These estimates were lower than in 2007 (6.9 million or 2.8 percent).

Among people who needed but did not receive illegal drug use treatment and felt they needed treatment (based on 2004 2005 combined data), the six most often reported reasons for not receiving treatment were (a) cost or insurance barriers (36.7 percent), (b) not ready to stop using (34.7 percent), (c) stigma (24.4 percent), (d) did not feel the need for treatment at the time or could handle the problem without treatment (16.3 percent), (e) did not know where to go for treatment (16.3 percent), and (f) other access barriers (16.1 percent).
