SMI in the past year was associated with past year substance dependence or abuse. Among adults aged 18 or older with SMI in 2008, 25.2 percent (2.5 million) were dependent on or abused illegal drugs or alcohol. The rate among adults without SMI was 8.3 percent (17.9 million).
Because of its chemical structure, heroin penetrates the brain more quickly than other opiates, which is probably why many opiate addicts prefer heroin.
Of those aged 12 or older who had not smoked cigarettes prior to the past year, the past year initiation rate for cigarettes was 2.7 percent in 2008, similar to the rate in 2007 (2.5 percent). Among teens aged 12 to 17 years who had not smoked cigarettes prior to the past year, the incidence rate showed no significant difference between 2007 (5.9 percent) and 2008 (6.2 percent). Among boys aged 12 to 17 who had never smoked prior to the past year, the decrease in the past year initiation rate from 6.1 percent in 2002 to 5.7 percent in 2008 was not statistically significant. Similarly for girls, the decrease from 7.4 percent in 2002 to 6.7 percent in 2008 was not statistically significant.

In 2002, the State with the lowest rate of perceived great risk of using marijuana occasionally (once a month) among persons aged 12 or older was Massachusetts (Table A.3). Only about 25 percent of all persons aged 12 or older in Massachusetts thought that occasional use was a great risk. This is striking in that Massachusetts reported only a moderate rate (6.3 percent) of past month use of marijuana in the 12 or older age group (Table A.2). States in the West and Northeast accounted for most of the lowest rates of perceptions of great risk of marijuana use among persons aged 12 or older. In addition to Massachusetts, there were Alaska, Colorado, Maine, Montana, New Hampshire, Oregon, Vermont, and Washington. Seven of the States having the lowest perceived risks also reported rates in the highest fifth for past month marijuana use for persons aged 12 or older. Similarly, of the 10 States indicating the highest perceived risk of occasional marijuana use among persons aged 12 or older, six had past month marijuana use rates that were in the lowest fifth (Alabama, Georgia, Mississippi, South Carolina, Texas, and West Virginia).
