Persons dependent on or abusing a substance in the past 12 months, or who received specialty treatment for a substance use problem within the past 12 months, are classified as needing treatment. In 2004, the number of persons aged 12 or older needing treatment for an alcohol or illegal drug use problem was 23.48 million (9.8 percent). Of these, 2.33 million received treatment at a specialty facility in the past year. Thus, 21.15 million people needed but did not receive treatment at a specialty facility in 2004. The number needing but not receiving treatment did not change significantly from 2002 to 2004.

Based on 2005-2008 combined data, among persons aged 12 or older who needed but did not receive illegal drug or alcohol use treatment, felt a need for treatment, and made an effort to receive treatment, the most often reported reasons for not receiving treatment were (a) no health coverage and could not afford cost (37.4 percent), (b) not ready to stop using (29.3 percent), (c) able to handle the problem without treatment (13.0 percent), (d) no transportation/inconvenient (10.6 percent), (e) no program having type of treatment (8.3 percent), (f) did not feel need for treatment at the time (8.2 percent), (g) did not know where to go for treatment (8.1 percent), (h) might cause neighbors/community to have negative opinion (7.7 percent), and (i) might have negative effect on job (7.4 percent).

Chronic drug abuse ultimately leads to a multitude of problems, including significant problems in relationships, school, and family functioning. Ironically, the social, legal, physical, financial, and emotional effects of substance abuse are usually worse than the original problem that the user was trying to avoid or to cope with.
