The States in the top fifth for needing but not receiving treatment for an illegal drug use problem were mainly in the West (five States) or in the Northeast (four States). California had the largest total number of persons aged 12 or older needing but not receiving treatment for an illegal drug use problem, approximately 798,000.

In 2008, the rate of current illegal drug use among persons aged 12 or older was higher in metropolitan areas than in nonmetropolitan areas. The rates were 8.5 percent in large metropolitan counties, 8.1 percent in small metropolitan counties, and 6.3 percent in nonmetropolitan counties as a group. Within nonmetropolitan areas, the rate was 7.2 percent in urbanized counties, 5.6 percent in less urbanized counties, and 6.1 percent in completely rural counties.

Illegal drug or alcohol dependence or abuse is associated with educational status. Among adults aged 18 or older in 2001, those who had not completed high school had the highest rate of illegal drug or alcohol dependence or abuse (8.2 percent), while college graduates had the lowest rate of illegal drug or alcohol dependence or abuse (6.1 percent). This association is due primarily to the strong association between illegal drug dependence or abuse with education (3.7 percent for adults with less than a high school education vs. 0.9 percent for college graduates). There is no clear association between education and alcohol dependence or abuse.

Both new and experienced users risk overdosing on heroin because it is impossible for them to know the purity of the heroin they are using. (Heroin sold on the street often is mixed with other substances such as sugar, starch, or quinine. An added risk results when heroin is mixed with poisons such as strychnine.) Heroin overdoses--which can result whether the drug is snorted, smoked, or injected--can cause slow and shallow breathing, convulsions, coma, and even death.

Looking at combined 2005-2006 data, rates of past month cigarette smoking were lower for pregnant women than nonpregnant women among those aged 26 to 44 (10.3 vs. 29.1 percent) and among those aged 18 to 25 (25.6 vs. 35.6 percent). However, among those aged 15 to 17, the rate of cigarette smoking for pregnant women was higher than for nonpregnant women (23.1 vs. 17.1 percent), although the difference was not significant. Similar patterns were observed in the combined 2003-2004 data.
