Drug and alcohol counseling that is administered within the context of a comprehensive treatment program will generally consists of four different components: Assessing the level of drug or alcohol usage and the rate of potential for addiction. In 2007, an estimated 2.7 million persons aged 12 or older used an illegal drug for the first time within the past 12 months. A majority of these past year illegal drug initiates reported that their first drug was marijuana (56.2 percent). Nearly one third initiated with psychotherapeutics (30.6 percent, including 19.0 percent with pain relievers, 6.5 percent with tranquilizers, 4.1 percent with stimulants, and 1.1 percent with sedatives). A sizable proportion reported inhalants (10.7 percent) as their first illegal drug, and a small proportion used hallucinogens as their first drug (2.0 percent).

After the initial effects, abusers usually will be drowsy for several hours. Mental function is clouded by heroin's effect on the central nervous system. Cardiac function slows. Heroin facts about its short term effects include severely slowed breathing, sometimes to the point of death. Both first time users and experienced heroin abusers are at risk for overdose because they never know the true purity of the heroin they are using. A heroin overdose can take place if the heroin the user has purchased is stronger than they anticipated or if the drug has been "cut" with a dangerous chemical.

In 2005, an estimated 19.7 million Americans aged 12 or older were current (past month) illegal drug users, meaning they had used an illegal drug during the month prior to the survey interview. This estimate represents 8.1 percent of the population aged 12 years old or older. Illegal drugs include marijuana/hashish, cocaine (including crack), heroin, hallucinogens, inhalants, or prescription-type psychotherapeutics used nonmedically.
