The percentage of persons estimated to be dependent on illegal drugs in the past year was about 65 percent of those who were estimated to be dependent on or abused illegal drugs in the past year. State estimates for illegal drug dependence for persons aged 12 or older ranged from 1.6 percent in Iowa to 3.0 percent in the District of Columbia. The highest rates of illegal drug dependence were among persons aged 18 to 25, with the highest rate in Massachusetts (8.3 percent).

An estimated 8.6 million people aged 12 or older (3.4 percent) were current users of illegal drugs other than marijuana in 2008. The majority of these (6.2 million persons or 2.5 percent of the population) used psychotherapeutic drugs nonmedically. An estimated 4.7 million persons used pain relievers nonmedically in the past month in 2008, 1.8 million used tranquilizers, 904,000 used stimulants, and 234,000 used sedatives.

Heroin ceases to produce feelings of pleasure in users who develop tolerance. These heroin users must continue taking the drug simply to feel normal.

In 2006, more than half of the 2.5 million persons aged 12 or older who received specialty substance use treatment in the past year also received treatment at a self help group (1.5 million persons). In addition, among those who received specialty substance use treatment, 377,000 received treatment at a prison or jail and 369,000 received treatment at an emergency room. The number who received treatment at a private doctor's office in 2006 was higher than the number in 2005 (422,000 vs. 254,000, respectively).

The top fifth for average annual incidence of marijuana for persons aged 12 or older was comprised mostly of States from the West (five) and from the Northeast (four). Seven States ranked in the top fifth for marijuana incidence in the 12 or older age group also ranked in the top fifth for past month marijuana use (Alaska, Colorado, Maine, Montana, New Hampshire, Rhode Island, and Vermont). Because most initiation of marijuana takes place at age 25 or earlier (Gfroerer, Wu, & Penne, 2002), the rates of initiation in the 26 or older age group were much lower than those in the 12 to 17 and 18 to 25 age groups: 0.1, 6.6, and 6.8, respectively. Vermont had the highest rate among teens aged 12 to 17 (10.4), while New Hampshire had the highest rate among persons aged 18 to 25 (11.1). In the 12 or older age group, seven States from the South were ranked in the lowest fifth for incidence of marijuana.
