Methadone maintenance treatment programs are generally staffed by professionals with medical and administrative expertise. The Nazis in Germany originally developed Methadone during war time in order to avoid the withdrawal symptoms that were associated with opium use, as their supply of the drug was quickly being cut off. Chemists developed Methadone, which not only worked, but its effects lasted a lot longer than the opium that it was developed to replace.

Treatment need is defined as having a substance use disorder or receiving treatment at a specialty facility (hospital inpatient, drug or alcohol rehabilitation, or mental health centers) within the past 12 months. In 2007, 23.2 million persons aged 12 or older needed treatment for an illegal drug or alcohol use problem (9.4 percent of persons aged 12 or older). Of these, 2.4 million (1.0 percent of persons aged 12 or older and 10.4 percent of those who needed treatment) received treatment at a specialty facility. Thus, 20.8 million persons (8.4 percent of the population aged 12 or older) needed treatment for an illegal drug or alcohol use problem but did not receive treatment at a specialty substance abuse facility in the past year.

Demand for safrole, a substance used in the manufacture of Ecstasy, is causing rapid and illegal harvesting of the tree which produces this substance in Southeast Asia, in particular the Cardamom Mountains in Cambodia.

Marijuana was the most commonly used illegal drug (14.6 million past month users). Among persons aged 12 or older, the rate of past month marijuana use was about the same in 2005 (6.0 percent) as in 2004 (6.1 percent), 2003 (6.2 percent), and 2002 (6.2 percent).
