An individual's perception of the risks of substance use has been shown to be related to whether he or she actually uses the substance (e.g., Bachman, Johnston, & O'Malley, 1998). In 2002, the State with the lowest rate of perceived great risk of using marijuana occasionally (once a month) among persons aged 12 or older was Massachusetts. Only about 25 percent of all persons aged 12 or older in Massachusetts thought that occasional use was a great risk. This is striking in that Massachusetts reported only a moderate rate (6.3 percent) of past month use of marijuana in the 12 or older age group. States in the West and Northeast accounted for most of the lowest rates of perceptions of great risk of marijuana use among persons aged 12 or older. In addition to Massachusetts, there were Alaska, Colorado, Maine, Montana, New Hampshire, Oregon, Vermont, and Washington. Seven of the States having the lowest perceived risks also reported rates in the highest fifth for past month marijuana use for persons aged 12 or older. Similarly, of the 10 States indicating the highest perceived risk of occasional marijuana use among persons aged 12 or older, six had past month marijuana use rates that were in the lowest fifth (Alabama, Georgia, Mississippi, South Carolina, Texas, and West Virginia).
Use of illegal drugs and alcohol was more common among current cigarette smokers than among nonsmokers in 2006, as in 2002 through 2005. Among persons aged 12 or older, 20.4 percent of past month cigarette smokers reported current use of an illegal drug compared with 4.2 percent of persons who were not current cigarette smokers (Figure 4.7). Past month alcohol use was reported by 66.3 percent of current cigarette smokers compared with 45.8 percent of those who did not use cigarettes in the past month. The association also was found with binge drinking (43.6 percent of current cigarette users vs. 16.1 percent of current nonusers) and heavy drinking (16.0 vs. 3.8 percent, respectively).
In 2005, there were 4.3 million persons aged 12 or older who had used alcohol for the first time within the past 12 months; this averages to approximately 12,000 initiates per day. The number of alcohol initiates was significantly greater than in 2002 (3.9 million), but similar to the numbers in 2003 (4.1 million) and 2004 (4.4 million).
Use of tobacco products other than cigarettes was higher among current cigarette smokers than among current nonsmokers. Approximately 1 in 20 current cigarette users (5.0 percent) reported using smokeless tobacco in the past month compared with 2.6 percent of nonsmokers. Moreover, 12.6 percent of current cigarette smokers also smoked cigars in the past month compared with 3.3 percent of those who did not smoke cigarettes.
In 2006, an estimated 22.6 million persons aged 12 or older were classified with substance dependence or abuse in the past year (9.2 percent of the population aged 12 or older). Of these, 3.2 million were classified with dependence on or abuse of both alcohol and illegal drugs, 3.8 million were dependent on or abused illegal drugs but not alcohol, and 15.6 million were dependent on or abused alcohol but not illegal drugs.
