Street heroin is widely varying and unpredictable in purity. This means that the user may prepare what they consider to be a moderate dose while actually taking far more than intended. There have been numerous reports of deaths by suicide that have been directly linked to the use of bath salts; this is most likely a result of the paranoia and extreme depression that is induced when individuals ingest these substances in order to get high.

The national rate in 2002.2003 for past year dependence on or abuse of alcohol or illegal drugs among persons aged 12 or older was about 9.2 percent. When examining dependence on or abuse of alcohol or illegal drugs at the State level, the States with high rates for alcohol dependence or abuse tended to dominate the top fifth for alcohol and illegal drug dependence or abuse combined because alcohol accounts for the vast majority of substance dependence or abuse. Seven States that ranked in the highest fifth for past year alcohol dependence or abuse also ranked in the top fifth for past year dependence on or abuse of alcohol or illegal drugs among persons aged 12 or older.

Among the 17.4 million people aged 12 or older who needed but did not receive treatment for an alcohol use problem in 2008, there were 651,000 (3.7 percent) who felt they needed treatment for their alcohol use problem. The number and the percentage were similar to those reported in 2007 (859,000 persons and 4.8 percent, respectively) and 2002 (761,000 persons and 4.5 percent, respectively). Of these, 512,000 (78.6 percent) did not make an effort to get treatment, and 139,000 (21.4 percent) made an effort but were unable to get treatment in 2008.
